  • Medical Case Management

    Confidential HIV Medical Case Management services are available to persons living with HIV/AIDS working toward leading an empowered life. HIV Medical Case Management services link HIV positive clients to health and social services. These services ensure clients with complex needs receive timely and coordinated services to function independently and productively.

    Our HIV medical Case Management is client centered. It’s focused on empowering the client with goal oriented, flexible, culturally sensitive, efficient, cost-effective, easily accessible, and collaborative efforts amongst the community of HIV providers in our service region.

    The development of a Care Plan allows clients to set and achieve tangible goals. Our trained staff provides coordination of medical, dental, legal, mental health, spiritual, social, financial, and home health care services. Other services include crisis intervention, advocacy, assistance with governmental programs, and referrals.

  • Financial Assistance

    In some cases, we can provide emergency financial relief to help with rent, utilities, medication, nutrition, transportation to medical appointments, and other essential needs. Eligibility for these services is determined by our agency guidelines. Clients must maintain regular contact with medical providers for us to assist.

  • Housing Assistance

    Stable housing is critical for our clients. PWA offers two comprehensive housing programs. The first is HOPWA (Housing Opportunities for People With AIDS) and allows PWA to get and keep clients in housing. This is through a voucher program in the counties we serve. The number of vouchers are limited, so clients who demonstrate the most need are referred to this program. Other funding through HOPWA allows us to provide limited assistance with rent, utilities and mortgage payments. Clients must attend regular medical appointments and remain treatment adherent. Funding through this program is available in our eight county service area.

    The second housing program focuses solely on individuals who are experiencing homelessness in Forsyth County. This program is made possible in collaboration with the City of Winston Salem and United Way of Forsyth County.
